Please fill out the Registration and New patient forms.  Kindly include means of identification (front and back of your driver’s license and if you are using insurance; include the front and back of the insurance card).  This will save you considerable time at the office.  If you are unable to do this, then please come at least 20 minutes prior to your appointment time to give you ample time to do your paperwork
