Please fill out the Registration and New Patient Forms.  Please include means of identification (front and back of your driver's license and if you are using insurance; please include the front and back of the insurance card).  This will save you considerable time at the office.  If you are unable to do this, then please come at least 20 minutes prior to your appointment time to give you ample time to do your paperwork.

We accept all types of payers.  However, we are both in network for some and out of network for other major insurance carriers. We recommend that you contact your insurance company to verify participation as our contracts change frequently with little notice from the insurance companies themselves.

Of note, even though we might be out of network for some insurance carriers, it does not mean that the cost of your labs, medications or surgery will be affected if Broward hospital or the lab where you do your blood work is in network with the insurance company.  The only difference is that the fee to the doctor who is seeing you in the office or doing your surgery will include a token. For example, the cost of doing a diagnostic laparoscopy is $300.  If you are in network, the insurance pays the physician the $300 after you have met your deductible.  If you are out of Network, the insurance company might decide to pay the doctor $250 and you will have to pay the doctor the other $50.

Call the office to make an appointment. However, we will need some information as to the services you are seeking. Send all your medical records, labs, and imaging reports to the office PRIOR to your appointment day, so the physician can review them before hand and be in a position to best advice you

If you are experiencing a life-threatening emergency, call 911 immediately. Prescription refills and routine questions are not considered a medical emergency and should be addressed Monday-Friday 10am-3pm. We do charge $100 for non-emergent questions after business hours, which is the patient’s responsibility as insurance companies do not cover non-emergency after hour care

Yes, we offer telemedicine services. Our provider will be able to consult with you at a more convenient time if you are unable to be seen physically.
